The sticky amyloid plaques are the fire. The brain’s own immune cells, called microglia, are the sprinklers. Their overreaction may be the real reason people with Alzheimer’s lose sleep.

Scientists have long blamed sleep loss on damaged neurons or the plaques themselves. A new study published in the journal Alzheimer’s & Dementia flips that idea. The real culprit appears to be inflammation driven by overactive microglia.

Microglia kept the brain awake like a party that never ends

Shannon L. Macauley, an associate professor of physiology, and first author Nicholas J. Constantino led the work. They studied two groups of mice. One group was genetically prone to developing amyloid plaques. The other group were wild type mice that aged normally. The team examined the animals at six months, when plaques first appear, and again at 18 months, which represents advanced disease.

The mice wore small head mounted devices that recorded brain activity through electroencephalography and electromyography. These tools allowed the researchers to track sleep patterns with precision.

What they found surprised them. The microglia were not just responding to the plaques. They were throwing an inflammatory response that kept the brain from getting deep, restorative sleep. Macauley described it as microglia partying all night and keeping the brain awake.

Removing microglia restored more than two hours of sleep

The researchers used a drug to temporarily eliminate most of the microglia. Even though the amyloid plaques remained unchanged, the mice regained more than two hours of sleep per day. This showed that the plaques themselves were not the direct cause of sleep loss. The immune reaction was.

Macauley called the finding paradigm shifting. It suggests that treating Alzheimer’s related sleep problems may not require clearing plaques. Calming the brain’s immune cells could be enough.

Why this matters for people with Alzheimer’s

Sleep loss is a common and distressing symptom of Alzheimer’s disease. It affects patients and their caregivers. Poor sleep can also make the disease worse, creating a damaging cycle. If scientists can find a way to calm overactive microglia in humans, they might restore sleep without needing to remove plaques.

The study was done in mice, not people. More research is needed to see if the same mechanism works in human brains. But the finding opens a new path for treatment. Instead of focusing only on plaques, researchers can now look at the immune system as a target for improving sleep and possibly slowing the disease.
