A 250 year old question about fluid in the brain

In 1776, Italian scientist Domenico Cotugno first described a strange phenomenon: fluid filled spaces in the brain that seemed to appear in people with dementia. For more than two centuries, no one could explain why those spaces formed or what they meant. Scientists at the Korea Advanced Institute of Science and Technology, known as KAIST, in Daejeon, South Korea, finally found the answer.

The team focused on a protein called aquaporin 4, or AQP4. This protein acts like a tiny water channel in brain cells. The researchers discovered that when AQP4 breaks down, fluid builds up in the brain. That buildup creates the spaces Cotugno saw. The condition is called brain edema, or swelling of the brain.

Why this matters for people with dementia

The study showed that in people with dementia, the AQP4 protein becomes damaged. Without properly working water channels, fluid cannot drain from the brain the way it should. The trapped fluid causes pressure and swelling. Over time, this damage may make dementia symptoms worse.

The research team used advanced imaging techniques to watch how fluid moved in the brains of both healthy people and those with dementia. They saw clear differences. In healthy brains, fluid moved freely. In brains affected by dementia, fluid pooled in certain areas. Those pools matched the spaces Cotugno described in the 18th century.

Local communities in South Korea have taken a strong interest in this work. Dementia rates are rising across the country, and families are looking for answers. The KAIST team has received attention from both medical professionals and the public.

The discovery does not offer a cure for dementia. But it gives researchers a new target for treatment. If scientists can find a way to protect or repair the AQP4 protein, they might be able to prevent the brain swelling that has been a mystery for 250 years. The work continues in Daejeon, where the team is now testing possible compounds that could keep the water channels working properly.
